Rosh Hashanah Health Questionnaire

Only one submission per person is required to attend all of your registered Rosh Hashanah services.

You must be registered and confirmed in order to attend High Holy Day services at Congregation Dor Tikvah. Please fill out and submit the Health Questionnaire below before arriving for services to confirm your good health and compliance with safety protocols.

Please arrive on time to services. The services are going to be shorter than usual and we do not want to increase exposure time by having to wait without enough people to begin. This is another way to express care and concern for your community.

Yom Kippur registration confirmations will be sent out early next week.
 

COVID-19 SAFETY RULES FOR CONGREGATION DOR TIKVAH 

  • All registered attendees must complete the Health Questionnaire safety screening. If you have registered a Plus 1, please have a separate form completed for them as well.

  • All persons are required to wear a mask at all times while on the Congregation Dor Tikvah property – surgical and cloth masks are acceptable.

  • Social distancing is to be observed at 8 feet between individuals. This applies to attendees on arrival and departure as well.

  • Machzorim (Prayer Books) will be provided at your seat, but, if you prefer, you are welcome to bring your own.

  • Someone will be designated to monitor compliance, ensure attendees are pre-registered, and record attendance in case follow up becomes necessary.

  • If someone becomes ill during services, they are to be isolated at once and arrangements are to be made for them to leave as soon as possible.

  • Children under the age of 10 years are currently not allowed to attend services. (This does not apply to the 3pm Sunday afternoon Family Shofar Mini Service.)

  • Congregants traveling outside of the Charleston area will need to wait for a 2-week period prior to attending services. Visitors to Charleston will be subject to the same waiting period upon their arrival. (This does not apply to the 3pm or 4pm Sunday afternoon Shofar Mini Services.)
